Why Wash Impressions Matter: A Step-by-Step Guide to Better-Fitting Dentures

Improve Fit. Reduce Adjustments. Increase Patient Satisfaction. When it comes to removable prosthetics, even the best digital workflow depends on one critical factor: an accurate impression.


Poor wash impressions are one of the leading causes of poor-fitting dentures, increased chair time, and remakes. Taking a wash impression (also known as a corrective or reline impression) with a record base and bite rim is an optional—but highly valuable—clinical step that helps ensure the final prosthesis accurately adapts to the patient's soft tissues while maintaining the established jaw relationship.

Although optional, this procedure provides the laboratory with a more accurate representation of the denture-bearing tissues under functional conditions. By investing just a few additional minutes during the record base and bite rim appointment, clinicians can improve the fit, stability, and comfort of the final prosthesis while reducing post-delivery adjustments.

Why Take a Wash Impression?

A wash impression refines the tissue surface of the record base after jaw relations have been established.

This additional impression helps:

•    Capture more accurate soft tissue anatomy 
•    Improve denture retention and stability 
•    Reduce pressure points and sore spots 
•    Minimize post-delivery adjustments 
•    Help the laboratory fabricate a more accurate final prosthesis 

Because the existing record base and bite rim maintain the patient's established vertical dimension of occlusion (VDO) and centric relation (CR), the wash impression enhances tissue adaptation without compromising the bite registration.

Step 1: Prepare the Record Base

Try in the record base and wax rim to verify fit, function, vertical dimension, and occlusion. Make any necessary adjustments before proceeding.

Inspect for undercuts, rough areas, or irregularities that could prevent complete seating or lock in the impression material. Relieve undercuts as needed while preserving stability. Confirm the record base seats fully without rocking and that the wax rim remains secure.

Clinical Tip: A stable, well-fitting record base is essential. Any movement during the wash impression can distort the tissue record and result in an ill-fitting prosthesis.

 

Step 2: Select the Impression Material

Choose a high-accuracy elastomeric impression material capable of reproducing fine soft tissue detail while maintaining dimensional stability.

Recommended materials include:

    • Light-body polyvinyl siloxane (PVS)
    • Medium-body polyvinyl siloxane (PVS)

These materials offer excellent flow characteristics, accurately capture soft tissue anatomy, and maintain dimensional accuracy during shipping and laboratory fabrication.

Follow the manufacturer's instructions for working and setting times. Prepare the dispenser gun and mixing tip immediately before loading to ensure a consistent, fully mixed material.

Clinical Tip: Avoid heavy-body materials, which may not flow adequately into vestibular extensions and can reduce the accuracy of the final tissue adaptation.


Step 3: Load the Record Base

Dispense the PVS material directly onto the tissue surface of the record base in a smooth, continuous layer.

Using the mixing tip or a suitable instrument, spread the material evenly across the entire intaglio surface while maintaining a consistent thickness. Ensure complete coverage of all tissue-bearing areas without trapping air bubbles.

Pay particular attention to the peripheral borders. Extend the material to the flange borders so the full depth of the vestibules can be recorded during border molding.

Before seating the record base, visually inspect the loaded impression material for any gaps, voids, or inconsistencies.

Key Objectives:

    • Apply a uniform layer of impression material.
    • Cover the entire tissue-bearing surface.
    • Extend material completely to all peripheral borders.
    • Maintain consistent thickness throughout the impression.

Clinical Tip: Overloading the record base can create excessive hydraulic pressure, displace the base during seating, and distort the final impression.


Step 4: Seat the Record Base and Capture Functional Anatomy

Seat the loaded record base using a straight, controlled path of insertion with gentle, even pressure. Avoid rocking or shifting after seating.

Ask the patient to perform functional movements such as:

    • Open slightly
    • Close gently
    • Smile
    • Pucker
    • Move the lips forward and side to side

At the same time, manually manipulate the:

    • Upper and lower lips
    • Buccal tissues
    • Cheeks

These functional movements shape the borders of the impression according to natural muscle activity and capture the vestibular anatomy under normal function.

Proper border molding improves the peripheral seal and contributes significantly to denture retention and stability.

Allow the impression material to polymerize completely before attempting removal.

Step 5: Remove and Inspect

After the impression material has fully set according to the manufacturer's recommended setting time, carefully remove the record base using a slow, controlled motion.

Inspect the completed wash impression under adequate lighting.

A clinically acceptable wash impression should demonstrate:

    • Sharp, well-defined tissue detail
    • Complete peripheral border extension
    • Uniform material thickness throughout
    • Accurate vestibular anatomy
    • No voids or air bubbles
    • No tears or pulls
    • No exposed areas (show-through) of the record base
    • Firm adhesion of the PVS material to the acrylic record base

If defects are present, repeat the procedure before submitting the case to the laboratory.

Clinical Tip: Small inaccuracies at this stage frequently translate into compromised denture adaptation and additional adjustment appointments after delivery.

Taking a few additional minutes to evaluate the completed wash impression can significantly reduce remakes, minimize adjustment appointments, and improve the overall fit and function of the final prosthesis. Accurate impressions provide the laboratory with the detailed information needed to fabricate a denture that delivers predictable clinical results and greater patient satisfaction.
